Abstract

lymph nodes may result in false positivity during imaging.4 Sestamibi scan has the highest sensitivity among noninvasive radiological techniques with the ability to localize mediastinal parathyroid glands.US is a cheaper technique and is free from radiation hazards.1,2,4,12 99m Tc-sestamibi single-photon emission computed tomography (SPECT)/CT has higher specificity for locating small adenomas and ectopic parathyroid. This imaging technique provides functional as well as anatomical details. 99m Tc-sestamibi SPECT/CT localizes the ectopic parathyroid in cases of doubt on planar images. However, CT or MRI can be used as an alternative in doubtful cases.2,9 CT and MRI both show great ability to detect ectopic parathyroid glands.Hematological reports in patients with PHPT may show hypercalcemia, raised or sometimes normal levels of PTH, low levels of phosphate and calcidiol, and an increased level of calcitriol.1,3,5,12 In cases of hypercalcemia with normal levels of PTH, other diagnoses should be sought.12 However, secondary HPT should be ruled out 1,
