Acute Retrocollis in the Emergency Department: A Diagnostic and Therapeutic Challenge
Abstract
This case study presents a case of a 27-year-old male with end-stage renal disease (ESRD) on maintenance haemodialysis who presented with acute retroflexion of the neck and upper limb dystonia following metoclopramide use.Neurological examination confirmed dystonia without focal deficits.Differential diagnoses such as tetanus, hypocalcaemia and central nervous system infections were ruled out.A diagnosis of metoclopramide-induced ADR was established, and the patient was successfully treated with intravenous diazepam, leading to complete symptom resolution. This case highlights the importance of recognising drug-induced dystonia in high-risk populations, ensuring timely intervention to prevent complications.
