Did you feel the earthquake?Please kindly complete the following macroseismic questionnaire.Your information will be used exclusively for research on earthquakes and seismic hazard in Bohemia, Moravia, and Silesia.We also ask you to provide the names of other persons who may supply additional information as observers, especially if they live in another municipality.Please always provide your address and telephone number, even if you did not feel the earthquake.We emphasize that every report, even the smallest one, is important for further processing.We are also interested in negative reports (see above) and in reports of earlier tremors, chronicle records, etc.Thank you for your willingness and cooperation, which allow us to obtain valuable data for the study of seismicity in the Czech Republic.
