PARTIAL HYDATIDIFORM MOLE ASSOCIATED WITH FETAL TRISOMY 21: DIAGNOSTIC CHALLENGES AND THE ROLE OF PLACENTAL MORPHOLOGICAL ANALYSIS
Abstract
Molar pregnancy (MP) is a gestational pathology resulting from abnormal fertilization, which leads to trophoblastic proliferation and the formation of either complete or partial molar forms. It represents the most common variant of gestational trophoblastic disease (GTD), which, in rare cases, may progress to gestational trophoblastic neoplasia (GTN).Partial molar pregnancy (PMP) occurs in approximately 1 in 700 pregnancies, although the true incidence may be higher due to insufficient histological verification in cases of spontaneous abortions and preterm Журнал «Перспективи та інновації науки» № 12(58) 2025 1919deliveries.This paper presents a rare clinical case of PMP combined with fetal trisomy 21, resulting in antenatal demise. The case illustrates the diagnostic challenges of this condition and highlights the essential role of placental histopathological examination in establishing the final diagnosis, particularly in the absence of typical clinical or ultrasound findings.Given the risk of persistent trophoblastic disease (0,5-1%), even in partial forms of MP, postpartum monitoring of β-HCG levels is recommended. This case underlines the importance of an integrated morphological, immunohistochemical, and cytogenetic approach in diagnosing gestational trophoblastic disorders and emphasizes the necessity of mandatory placental examination in cases of fetal demise or congenital anomalies.
