The Bio-Energetic Dependency of Glymphatic Clearance: A Causal Link Between TOMM40 Variants and AQP4 Depolarization in Alzheimer's Disease
Abstract
Abstract Despite decades of investment, anti-amyloid therapies have delivered only marginal clinical benefit, underscoring a fundamental limitation of Alzheimer’s disease (AD) models centered on protein production rather than clearance. This failure compels a conceptual shift toward the mechanisms by which the brain removes metabolic and proteotoxic waste. Here, we propose a unifying hypothesis: glymphatic clearance is a strictly bio-energetic process, and its failure represents a primary driver of neurodegeneration. At the cellular level, efficient glymphatic flow depends on the polarized localization of Aquaporin-4 (AQP4) to astrocytic perivascular endfeet. This polarization is not passive. It requires sustained ATP availability to support actin cytoskeletal dynamics, vesicular trafficking, and anchoring complexes that maintain endfoot architecture. Astrocyte endfeet constitute one of the most energy-intensive microdomains in the brain, integrating ion homeostasis, water flux, and vascular coupling. We posit that while basal ATP is sufficient for cell survival, subtle bio-energetic deficits lead to the progressive destabilization of actin-dependent AQP4 polarization, functionally obstructing interstitial fluid clearance. We identify TOMM40 poly-T length variants as a critical upstream driver of this energetic vulnerability. TOMM40 encodes the central pore of the mitochondrial protein import machinery. We hypothesize that regulatory variants reduce the effective import flux of mitochondrial proteins, creating a chronic kinetic bottleneck. This results in reduced spare respiratory capacity and localized ATP insufficiency in astrocytic endfeet. Crucially, TOMM40 variants act as energetic risk modifiers—lowering the bio-energetic threshold for failure rather than acting as deterministic disease drivers. This mechanism does not increase amyloid production but instead prolongs its residence time by impairing clearance. Finally, we propose a rational therapeutic strategy focused on sequential metabolic rescue. We argue that NAD⁺ precursors must first restore mitochondrial proteostasis via SIRT3-mediated activation of the mitochondrial unfolded protein response (UPR^mt). Subsequently, ketone bodies provide an efficient bioenergetic substrate to supply the ATP surplus required to re-establish actin integrity. In this framework, Alzheimer’s disease emerges not primarily as a protein aggregation disorder, but as a metabolic plumbing failure driven by bio-energetic collapse at the neurovascular interface.
