Abstract

Position Paper (Part1)Abstract:In contemporary society, discussions of transgender people and gender diversity have increasingly centered on protection, respect, and social inclusion. While this shift has yielded meaningful progress, historical memories of eugenics and conversion therapy—along with ongoing ethical concerns—have often led to inquiries into the underlying mechanisms of gender incongruence, including gender dysphoria, being framed as dangerous or inherently negative. Against this backdrop, this essay examines the ethical and clinical implications of mechanistic inquiry in transgender healthcare and human-rights discourse. In particular, current hormone therapy is primarily organized around safety and standardization. Yet even under similar treatment conditions, inter-individual differences in response are repeatedly observed, revealing interpretive limits within existing clinical frameworks. This paper argues that biological and developmental mechanistic understanding should not function as a prerequisite for defining, restricting, or “correcting” treatment. Rather, it can serve as an auxiliary interpretive framework—one that helps clinicians and patients more carefully interpret heterogeneity and uncertainty observed in practice. From this perspective, mechanistic discussion need not be an attempt at pathologization; it can serve as an ethical and clinical foundation for pursuing safer and more predictable approaches to hormone therapy.
