Abstract

This N-of-1 observational study documents a successful case of omeprazole discontinuation using a patient-led, symptom-guided tapering strategy. While traditional clinician-recommended tapering schedules (alternating PPI/H2RA doses) failed due to intolerable rebound acid hypersecretion (RAH), a granular approach involving 5-10% weekly dose reductions—facilitated by weighing enteric-coated granules—permitted complete cessation over 162 days. The findings highlight the potential for flexible, fine-grained tapering to manage RAH in clinical practice, even in low-risk patients.
