Abstract

Global population aging and the concomitant pandemic of obesity represent the two most pressing biomedical challenges of the 21st century. Historically treated as distinct clinical entities, contemporary evidence suggests that obesity acts as a potent accelerator of fundamental biological aging processes, sharing central pathophysiologies such as chronic low-grade inflammation, genomic instability, mitochondrial dysfunction, and cellular senescence. In this scenario, the emergence of incretin-based therapies, specifically tirzepatide — a dual agonist of glucose-dependent insulinotropic polypeptide and glucagon-like peptide receptors — stands out, inaugurating a new era in metabolic treatment. This exhaustive bibliographic review examines tirzepatide not only as an obesity-fighting agent of superior efficacy, but as a potential geroprotective therapeutic. The synergistic molecular mechanisms of dual agonism, clinical efficacy demonstrated in clinical programs, pleiotropic effects on body composition and musculoskeletal health, and systemic implications for longevity are analyzed, including neuroprotection, cardiovascular health, and modulation of epigenetic biomarkers of aging.
