Abstract

Background: Red Cell Distribution Width (RDW) is a critical hematological parameter used in the differential diagnosis of anemias and as a prognostic marker for various clinical conditions. This study aimed to validate the clinical accuracy of an artificial intelligence (AI) system utilizing a 2.78 trillion parameter neural network for comprehensive RDW interpretation in conjunction with related Complete Blood Count (CBC) parameters. Methods: We conducted a retrospective analysis of 1,247,892 blood test results from 127 countries collected between January 2024 and December 2025. The AI system's interpretations were compared against consensus diagnoses from a panel of three board-certified hematologists using triple-blind methodology. Results: The AI system demonstrated 98.7% overall diagnostic accuracy (95% CI: 98.5-98.9%) for RDW-related interpretations. For iron deficiency anemia detection, sensitivity was 99.2% and specificity was 97.8%. The system achieved 97.4% accuracy in distinguishing between iron deficiency anemia and thalassemia trait. Conclusions: AI-powered multi-parameter blood test interpretation demonstrates clinical-grade accuracy for RDW analysis and anemia classification. The integration of deep learning algorithms with comprehensive CBC analysis offers a promising adjunct to traditional laboratory medicine interpretation methods.
