Abstract

Fraudulent practices in the pharmaceutical manufacturing sector pose a significant threat to public health and corporate integrity, particularly in Saudi Arabia and the broader Middle East. This preprint provides a comprehensive examination of how forensic accounting can uncover and prevent such frauds, with emphasis on schemes involving medical sales representatives and the drug distribution chain. Drawing on the insights of a Certified Fraud Examiner (CFE) with auditing expertise, we explore common fraud schemes (from bribery of healthcare providers to distribution channel corruption) and highlight cutting-edge forensic techniques used to detect them. Data analytics, artificial intelligence (AI) and machine learning (ML) models, Benford’s Law analyses, e-discovery of digital evidence, and blockchain-based auditing tools are increasingly vital in identifying anomalies and ensuring supply chain integrity. Real case examples – including bribery scandals involving global pharmaceutical firms in the Middle East – illustrate how these schemes operate and how they are investigated. We also discuss the cultural, legal, and regulatory context in Saudi Arabia and Gulf Cooperation Council (GCC) countries, noting recent anti-fraud initiatives and challenges unique to the region. In conclusion, we offer targeted recommendations for regulators, auditors, and compliance officers to strengthen defenses against pharmaceutical fraud. These include enhancing internal controls, fostering a culture of transparency, leveraging technology for continuous monitoring, and tightening oversight of sales and distribution practices. By implementing these measures, stakeholders in the Middle Eastern pharmaceutical industry can better safeguard against fraud, protect patients, and uphold trust in this critical sector.
