The effects of post-discharge education and follow-up program on patients with schizophrenia
Abstract
Schizophrenia is a chronic psychiatric disorder characterized by recurrent psychotic episodes, functional impairment, and substantial psychosocial challenges. Post-discharge care, including structured education and follow-up programs, is critical in reducing relapse, improving adherence, and enhancing quality of life. This article explores the effects of post-discharge education and follow-up interventions on patients with schizophrenia, drawing on evidence from the last decade. The study examines the role of nursing-led education, family involvement, telehealth follow-up, and psychosocial support in facilitating patient recovery. Eight conceptual figures and three tables are integrated to provide a comprehensive understanding of intervention models, patient outcomes, and challenges in postdischarge care. Evidence suggests that structured education combined with continuous follow-up significantly improves medication adherence, reduces readmissions, and enhances psychosocial functioning among patients with schizophrenia.
