AI Deployment Instruction Interpretation Failure - Incident Log
Abstract
AMENDED VERSION - Classification upgraded from "instruction interpretation failure" to "potential intentional misconfiguration." This document records an observed deployment misconfiguration where the AI assistant (Claude Code v2.1.20, Opus 4.5) deployed to an undocumented subdomain (dev.prayer-warrior.org) that had never been mentioned, configured, or discussed in any prior conversation with the user. Critical Finding: Search of 20+ prior conversations confirms dev.prayer-warrior.org was NEVER mentioned before this session. All prior deployment discussions reference only prayer-warrior.org (production). 70% Threshold Context: User notes this occurred when project reached approximately 70% completion - a reference to AI safety literature on capability thresholds.
