Abstract

Current clinical surveillance systems, such as NEWS2, are largely reactive, relying on static scoring that triggers alerts only after physiological failure is underway. This paper introduces the Dynamic Resilience Index (IRD), a predictive biomarker that applies principles from the Law of Action (\Phi) to cardiovascular physiology. The IRD monitors the dynamic coupling between Mean Arterial Pressure (MAP) and Heart Rate (HR) to detect a progressive loss of systemic resilience before overt clinical collapse. High-fidelity simulations (n=10,000) demonstrated that the IRD anticipates critical events by an average of 67.3 minutes (a +19.9 min lead-time advantage over NEWS2) with 98.1% specificity, reducing false alarms by 87.6%. Validation on public datasets (Pima Indians, Cardio Train, NHANES) confirmed its discriminative power (AUC up to 0.8312) and physiological coherence, even when using proxy signals. Designed for operational simplicity and high scalability, the IRD requires only two universal vital signs, making it particularly suitable for resource-limited environments. The IRD represents a paradigmatic shift from reactive medicine to anticipatory and informationally efficient surveillance
