This paper proposes a non-diagnostic, log-structured, multi-branch questionnaire frameworkdesigned to reduce medical decision errors caused by unstructured patient-reported information.The framework organizes self-reported data across temporal, behavioral, and response-based dimensionsand generates multiple outputs from identical inputs, including patient-oriented summaries,clinician-oriented contextual views, and safety-oriented “do-not-do” signals. The proposed system does not perform diagnosis, treatment, or prescription.It operates solely as a pre-decision safety layer to support professional judgmentby identifying informational risks, contradictions, and uncertainty patterns.
