Abstract

This study examines the transformation of governance in the implementation of the tuberculosis (TB) control program as a public service through a contemporary public administration framework. Covering the January 2025–January 2026 implementation period, the research analyzes how national TB policy is translated into operational practice, focusing on active screening, treatment adherence, preventive therapy, central local coordination, community engagement, and administrative evaluation mechanisms. Using a qualitative descriptive approach grounded in public administration research traditions, the study employs documentary triangulation and practice-based analysis of policy documents, technical guidelines, and implementation reports. The findings demonstrate that program effectiveness is shaped by organizational capacity, collaborative governance, and indicator-based monitoring systems. Key constraints include resource limitations, administrative complexity, and persistent social stigma, which affect service continuity and patient adherence. The study contributes to governance and health service literature by positioning TB program implementation as an adaptive administrative system that integrates institutional coordination, procedural standardization, and community legitimacy. Policy recommendations emphasize strengthening human resource capacity, standardizing operational procedures, expanding social support mechanisms, and developing integrated monitoring and evaluation frameworks. These insights support evidence based governance reform aimed at improving the sustainability and accountability of public health service delivery.
