Audit Failure Modes in Post-Hoc Reconstruction of Execution Evidence
Abstract
This technical brief documents a recurring audit failure mode in which post-hoc reconstruction is used to substitute for contemporaneous execution evidence. It explains why reconstructed logs, screenshots, attestations, and narrative representations fail established audit thresholds for independence, contemporaneity, and chain-of-custody, and why such failures are treated as evidentiary insufficiency rather than curable documentation defects. The brief outlines an emerging replacement pattern based on execution-time evidence objects generated and accepted at the moment of execution, enabling objective, replayable verification for audit, compliance, and reliance purposes.
