Abstract

This document presents the Strategic Research and Innovation Agenda (SRIA) and Roadmap developed within the PROPHET initiative, addressing the emerging paradigm of personalised prevention in healthcare. Advances in sequencing and genotyping technologies, combined with the integration of digital health resources, have enabled the identification of individualised risk profiles based on genetic, biological, behavioural, environmental, socio-economic and cultural factors. Personalised prevention aims to prevent disease onset, progression and recurrence through targeted, timely, effective and equitable interventions, with the goal of improving quality of life and reducing the burden on healthcare systems. While several implementation challenges remain, genomics represents the most advanced field, with established clinical applications such as genetic testing, polygenic risk scores and pharmacogenomics. The SRIA analyses the complexity of personalised prevention and highlights its importance for European healthcare systems. Based on a review of the scientific literature and the perspectives of multiple stakeholders, ten key challenges are identified, including data infrastructure and integration, evidence generation, community engagement and trust, professional and policy involvement, regulatory and ethical issues, access and equity, and behaviour change. The accompanying Roadmap provides a structured framework for implementation, defining goals, priority actions, timelines, responsible actors, funding sources and synergies with European initiatives. In total, 56 goals and 66 actions are proposed, with an expected implementation timeframe of five years and a focus on achieving long-term impact across Europe.
