ROLE OF HLA-ALLELES MAJOR HISTOCOMPATIBILITY COMPLEX AND MULTIPLE MYELOMA CLINIC, DIAGNOSIS, TREАTMENT
Abstract
The article presents the results of retrospective and prospective studies of immunogenetic characteristics of the major histocompatibility complex (MHC), typed for the first time according to the human leukocyte antigens (HLA) tissue compatibility system - alleles of the I class of loci (A and B)* and II class of the locus DRB1*of patients with multiple myeloma (MM) of the indigenous people, inhabitants residents of the Kyrgyz Republic (Kirghizia). A comparative assessment of the incidence of HLA-antigens among patients with multiple myeloma of kirghiz nationality and patients with multiple myeloma of the Russian-speaking population (mixed nation and different nationalities) of the indigenous inhabitants of the Kyrgyz Republic (Kirghizia) was carried out, while there are quite highly reliable significant differences. The most common antigens are HLA-А10, HLA-А25, HLA-А28, HLA-В10, HLA-В13, HLA-В14, HLA-В17, HLA-В40, HLA-В58 and HLA-DRB1*02, HLA-DRB1*09, HLA-DRB1*14, HLA-DRB1*16 among patients multiple myeloma kirghiz nationality. In patients with identified antigens HLA-A10, HLA-A25, HLA-В13, HLA-В17and и HLA-DRB1*04, HLA-DRB1*11, HLA-DRB1*14, HLA-DRB1*16, the clinical picture proceeds with an aggressive course of the disease and a high risk of progression. When using a high-tech treatment method, take into account the identified significant HLA-markers in patients with multiple myeloma of kirghiz nationality. Thus, the major histocompatibility complex of HLA-alleles in the body's immune system plays a huge role, and it is important to search for immunological, immunogenetic criteria for predisposition to diseases, study and timely diagnosis of immune protection factors in a patient, in particular mucosal immunity leading to severe inflammatory diseases, complications with damage to organs and systems, development of a secondary immunodeficiency state. An early study will identify the impact of imperceptibly, gradually external and internal factors that subsequently affect the process of occurrence, development, course, treatment and prognosis of the disease.
