Abstract

Leech therapy, or Hirudotherapy, is an ancient healing practice that has regained attention in modern medicine for its therapeutic potential. Historically used for bloodletting and detoxification, it is now understood to work through a complex mixture of bioactive compounds in leech saliva, such as hirudin, calin, and bdellins. These substances exhibit anticoagulant, anti-inflammatory, analgesic, antioxidant, and antimicrobial properties, making leech therapy a versatile treatment option. Research shows its effectiveness in enhancing blood circulation, promoting wound healing, reducing chronic inflammation, and supporting tissue repair. Clinically, it has proven valuable in reconstructive and microsurgical procedures, the management of venous congestion, inflammatory disorders, and various dermatological conditions. Despite its benefits, safety considerations and contraindications remain important, as inappropriate use can lead to complications. Current limitations also include the absence of standardized treatment protocols and insufficient large-scale clinical trials to confirm long-term outcomes. Nevertheless, leech therapy represents a unique integration of traditional medical wisdom with contemporary scientific evidence. Its re-emergence highlights the potential of natural biological agents in addressing complex health challenges. While promising, further clinical investigations are needed to better define its mechanisms, ensure safety, and optimize therapeutic guidelines. Hirudotherapy ultimately reflects the successful convergence of ancient practices with modern biomedicine, offering new possibilities in complementary and integrative healthcare.
