Reframing Historically Documented Failures: A Coordinated Series of Legibility-First Comparative Papers and Methodological Supplement
Abstract
This Zenodo deposit presents a coordinated research series applying a legibility-first interpretive framework to historically documented failures across engineering, public health, finance, criminal justice, institutional governance, disaster response, and evidentiary systems. The series is explicitly bounded. It does not claim to replace domain expertise, to offer a universal predictive model, or to collapse all failures into a single causal account. Its narrower contribution is comparative and diagnostic: it tests whether a repeatable analytical sequence centered on accepted representation, warning uptake, threshold governance, and continuation or closure can clarify structural patterns that domain-specific narratives often leave only partially visible. The deposit includes a methodological supplement, a series overview and reading-order file, and companion application papers. The methodological supplement defines the framework’s core operational terms, distinguishes technical or measurement, institutional or organizational, and social or narrative forms of accepted representation, clarifies threshold ownership and intervention logic, addresses hindsight risk and reproducibility concerns, and includes stress-test cases where the framework has weaker or more ambiguous payoff. The companion papers then apply the framework to historically documented case clusters across multiple domains. Recommended reading order: begin with 00_Series_Overview_and_Reading_Order.docx, then 07.Methodological Supplement and Stress-Test Cases for Reframing Historically Documented Failures.docx, then 07.Reframing Historically Documented Failures part 1_hardened.docx, then Parts 2–13 as companion application papers. File index 00_Series_Overview_and_Reading_Order.docx — Overview of the deposit, reading order, series contents, and relation to the canonical framework backbone. 07.Methodological Supplement and Stress-Test Cases for Reframing Historically Documented Failures.docx — Methodological supplement defining core operational terms, representation types, threshold logic, sequence logic, reproducibility guidance, hindsight controls, and a nine-case stress-test set. 07.Reframing Historically Documented Failures part 1_hardened.docx — Challenger, Therac-25, and 737 MAX; benchmark paper and main theory-positioning application. 07.Reframing Historically Documented Failures part 2.docx — Air France 447, Three Mile Island, and Ariane 5 Flight 501; observability and signal-loss failures. 07.Reframing Historically Documented Failures part 3.docx — Flint, Deepwater Horizon, and Bhopal; environmental and institutional failures. 07.Reframing Historically Documented Failures part 4_hardened.docx — Flash Crash, Knight Capital, and TSB’s 2018 migration incident; financial and software failures. 07.Reframing Historically Documented Failures part 5.docx — George Floyd, Breonna Taylor, and Tyre Nichols; fatal-force cases and threshold failure in de-escalation or interruption. 07.Reframing Historically Documented Failures part 6.docx — Kitty Genovese, the Central Park Five, and Richard Jewell; narrative lock-in and premature closure. 07.Reframing Historically Documented Failures part 7_hardened.docx — I-35W bridge collapse, Mid Staffordshire, and Silicon Valley Bank; masked stability and false equilibrium. 07.Reframing Historically Documented Failures part 8_hardened.docx — Tenerife, Texas City, and Fukushima Daiichi; compensatory burden and overburdened-node failure. 07.Reframing Historically Documented Failures part 9_hardened.docx — Therac-25, 737 MAX after Lion Air, and Wells Fargo; revision failure and continuity-preserving misupdate. 07.Reframing Historically Documented Failures part 10_hardened.docx — Financial crisis, Atlanta Public Schools cheating scandal, and Robodebt; measurement, calibration, and scoring failure. 07.Reframing Historically Documented Failures part 11_hardened.docx — Brandon Mayfield, Iraq WMD intelligence, and Annie Dookhan; provenance distortion and evidentiary lineage collapse. 07.Reframing Historically Documented Failures part 12_hardened.docx — Chicago heat wave, Hurricane Katrina, and Texas winter storm; silo failure and translation breakdown. 07.Reframing Historically Documented Failures part 13_hardened.docx — Hillsborough, Penn State, and the Post Office Horizon scandal; forced coherence and overwrite. Taken together, the series is intended as a portable comparative framework for retrospective interpretation under uncertainty. Its strongest claim is not that it discovers every relevant failure mechanism anew, but that it preserves a disciplined sequence for comparing how representation degrades, how warning fails to become action-governing, how thresholds do or do not activate, and how continuity or closure persists under unresolved danger. The included stress tests also help mark the framework’s current boundary conditions by identifying cases where its payoff is limited, ambiguous, or secondary to other explanatory structures.
