Synergistic Pharmacological Repurposing in Autosomal Dominant Polycystic Kidney Disease (ADPKD): An N=1 Clinical Auto-Report of Pentaconjugate Therapy.
Abstract
Abstract Background: Autosomal Dominant Polycystic Kidney Disease (ADPKD) remains a challenge in clinical nephrology, particularly in cases with aggressive genetic phenotypes. Standard therapy often targets symptoms rather than the underlying multi-pathway signaling responsible for cystogenesis. This document transcends traditional clinical observation, establishing a bridge entre el rigor farmacológico y la integración mente-cuerpo. Methods: This N=1 clinical auto-report details a high-level, pentaconjugate pharmacological protocol meticulously designed by a 32-year-old clinical pharmacist with a confirmed aggressive familial PKD1 phenotype. The selection of this therapeutic arsenal is not random; it is the result of an exhaustive objective evaluation of the molecular pathophysiology of the disease. The protocol synergistically targets mTOR, HDAC, and cAMP pathways while integrating neuro-endocrine allostatic modulation. Baseline Results: At Month 0, the subject presents with an eGFR of 60 mL/min/1.73 m² and a stable blood pressure of 120/80 mmHg. The intervention addresses historical sympathetic triggers (phentermine/muscle strain) through a comprehensive somatic and pharmacological shift. Conclusion: This framework provides a rigorous foundation for evaluating long-term renal preservation through multi-target inhibition in high-risk ADPKD patients, elevating the discourse from mere symptom management to a paradigm of elite therapeutic control.
