Health 4.0: The System of Care — A Blueprint for American Health Security
Abstract
The United States does not lack medical talent, scientific capacity, or technological possibility. It lacks a system of care capable of securing health across a lifetime. American healthcare remains organized around episodes, institutions, payers, rescue, billing codes, and administrative permission. The result is a system that spends more than peer nations, produces worse aggregate outcomes, imposes avoidable financial risk on households, fragments clinical responsibility, and leaves people to rebuild their story at every transition of care. This paper specifies Health 4.0 as a system-of-care blueprint for American health security. It translates the Trajectory Engineering framework developed in Papers I and II into a practical care architecture organized around two linked modes: Pulse Health and Rescue Health. Pulse Health is the continuous, upstream, longitudinal system that keeps a person's health trajectory visible and correctable before crisis. Rescue Health is the acute, specialty, procedural, emergency, and complex-care system that remains indispensable when crisis, injury, advanced disease, or high-intensity intervention is required. Health 4.0 does not replace Rescue Health. It surrounds it with Pulse Health so that Rescue is reserved, as much as possible, for what cannot be prevented, anticipated, or stabilized upstream. The operating relationship of Health 4.0 is the Triangle of Trust: the Human Principal, the Human Health Ally, and Doctor AI. The Human Principal is the person whose body, biography, culture, data, finances, and future are at stake and is the seat of authority. The Human Health Ally is the licensed human professional who brings clinical judgment, fiduciary duty, conscience, and relationship. Doctor AI is the longitudinal, language-concordant operating layer. The Human Health Ally is on the loop, not in every loop. This is No. III in The Trajectory Engineering Papers.
