A Bipolar Pathological Model of Cervical Kyphosis (Straight Neck): A New Classification Based on Qi Ascending-Descending Dynamics, Shaoyang Pivot Function, and Diaphragmatic Coupling
Abstract
This paper presents a conceptual framework that reinterprets cervical kyphosis — commonly known as "straight neck" — through the lens of Oriental Medicine Neurodynamics (OMN™), a clinical framework grounded in classical East Asian medicine. While contemporary orthopedic medicine predominantly attributes straight neck to external factors such as prolonged smartphone use and poor postural habits, this paper proposes that posture is not a cause but a result of disrupted Qi ascending-descending dynamics. Two distinct pathological types are identified: Type I (Deficiency), characterized by Heart Qi and Kidney deficiency with loss of structural support; and Type II (Excess), characterized by Wood Qi excess and Gallbladder channel hypertonicity with active muscular over-contraction. The paper further explores the physiological basis of the classical concept "Shaoyang as Pivot" (少陽為枢, Líng Shū, Chapter 5) in relation to diaphragmatic dynamic tension, presents a comparative analysis with Wilhelm Reich's Muscular Armor theory, and examines the diaphragm's role as a gateway in Qi circulation. This paper is based on over 30 years of clinical observation and presents a conceptual-clinical hypothesis model requiring further empirical verification.
