Abstract

Oncoplastic breast surgery integrates oncological surgical principles with plastic surgery techniques, ensuring wide, clear resection margins while achieving optimal cosmetic outcomes. Widely adopted in recent years, oncoplastic breast surgery has been proven oncologically safe and allows for simulta-neous or delayed symmetry correction of the contralateral breast. Techniques are categorized as Level I or Level II, based on the extent of tissue removal. Among Level I techniques, Batwing and Hemi-batwing mammoplasty offer effective volume displacement and are suitable for central and upper quadrant breast tumors. The Batwing technique, characterized by batwing-shaped incisions, enables tumor resection while preserving the nipple-areola complex unless oncological indications necessitate its removal. Hemibatwing mammoplasty, a variation involving a single triangular incision, is ide-al for lateral periareolar tumors. Both methods ensure oncological safety and cosmetic satisfaction, with advantages such as short operative times, low complication rates, and applicability for benign or malignant tumors in selected patients. The surgical procedure involves preoperative marking, tu-mor excision, tissue mobilization, multilayer closure, axillary procedures, and optional contralateral breast symmetrization. Modifications of the original technique, such as adjusted incisions or potential excision of the nipple-areola complex, contribute to the application of this technique in selected pa-tients. Postoperative radiation therapy remains integral, requiring precise tumor bed localization and collaboration between surgeons and radiation oncologists to optimize treatment planning. Patient sat-isfaction with oncoplastic breast surgery is high, as it combines oncological treatment with improved quality of life and aesthetic outcomes. Tools like the BREAST-Q questionnaire measure psychosocial and cosmetic benefits. Studies highlight significant improvements in patient-reported outcomes and psychological well-being, although physical discomfort may increase post-surgery. Cost-effective in the long term, Batwing and Hemibatwing mammoplasty reduce the need for subsequent reconstructive surgeries and mitigate psychological distress, making them valuable options in breast cancer treatment.
