ONCOPLASTIC SURGERY TECHNIQUES- J-V MAMMOPLASTY
Abstract
Oncoplastic Breast Surgery (OPS) has become an established standard in the treatment of breast can-cer, aiming to address both oncological safety and aesthetic outcomes. The primary objectives of OPS include correcting breast ptosis or macromastia, achieving durable breast contour and symmetry, pre-serving function and sensation, and ensuring cosmetically acceptable scarring. The selection of an appropriate reduction mammoplasty technique depends on several factors, including previous surgical scars, tumor location, and the desired postoperative breast volume. In patients with markedly large and ptotic breasts who are not ideal candidates for nipple-sparing mastectomy, skin-reducing mastectomy employing either J-scar or V incision patterns can be considered. The J-mammoplasty approach is particularly suitable for tumors located in the lower outer quadrant—specifically, at the 4-5 o’clock position in the left breast and 7-8 o’clock in the right breast. Conversely, the V-mammoplasty technique is more appropriate for tumors situated in the lower inner quadrant, corresponding to the 7-8 o’clock position in the left breast and 4-5 o’clock in the right breast.
