SKIN REDUCING MASTECTOMY (IN PATIENTS WITH LARGE AND PTOTIC BREASTS)
Abstract
Skin-reducing mastectomy (SRM) with immediate implant-based reconstruction is a safe oncologi-cal procedure that can achieve excellent aesthetic outcomes in patients with large and ptotic breasts. Implants may be placed either in the prepectoral or retropectoral plane, depending on patient-specific factors and surgeon preference. Nipple preservation is feasible through various surgical techniques when there are no oncological contraindications. Proper patient selection and technique are critical to ensure the safety of nipple-sparing approaches. Early complications primarily involve tension at the T-junction of the Wise pattern closure or surgical site infection. Long-term complications are similar to those associated with other implant-based reconstruction methods, including capsular contracture, im-plant malposition, or rupture, and are managed accordingly. Optimal outcomes require careful patient selection, meticulous surgical technique, and comprehensive preoperative planning.
