ONCOPLASTIC SURGERY TECHNIQUES - LATERAL (RACKET) MAMMOPLASTY
Abstract
Oncoplastic surgery integrates oncological resection and plastic surgery techniques to achieve optimal cancer control while maintaining aesthetic outcomes. Racket handle mammoplasty, also referred to as lateral mammoplasty, is a Level II oncoplastic technique designed for tumors localized in the lateral breast quadrants. This method combines oncological safety with breast reshaping to optimize cosmetic outcomes. The procedure involves positioning the patient in a semi-sitting position with the operated breast side elevated to ensure optimal visualization of the operating field and ergonomic access for the surgeon. The surgical steps include circular incisions around the nipple-areola complex (NAC), curving lateral incisions for tumor resection, and axillary intervention when necessary. Reconstruction involves approximation of the tumor cavity to restore the natural contour of the breast, ensuring both oncological safety and aesthetic goals are met. Racket handle mammoplasty is especially advantageous for patients with moderate to large breast sizes or mild to moderate breast ptosis, allowing for larger resections with clear margins. This technique also provides an opportunity for contralateral symme-trization when needed. Despite requiring advanced surgical skills, this approach achieves superior functional and aesthetic outcomes, improving quality of life for breast cancer patients.
