A Case of Late Esophageal Stenosis Due to Cytomegalovirus Infection
Abstract
A 20-year-old female patient presented with progressive dysphagia over the past 3 months. She had a history of leukemia and bone marrow transplant, with no immunosuppressive therapy in the last 2 years. Endoscopic examination revealed an ulcerated stricture in the midesophagus, and biopsy showed positive cytomegalovirus (CMV) nuclear staining. Ganciclovir therapy was initiated, leading to significant clinical improvement. Follow-up endoscopy demonstrated a marked reduction in the stricture. This case highlights the possibility of late-onset CMV-related esophageal pathology even after the cessation of immunosuppressive therapy. Clinicians should be aware of CMV as a possible etiologic factor for esophageal strictures in patients with a history of immunosuppression, even years after treatment cessation.
