Abstract

Background: Perinatal palliative care (PPC) has emerged as a critical subspecialty within modern neonatology, necessitated by the increasing complexity of prenatal diagnostics and the subsequent identification of life-limiting fetal conditions. As medical technology pushes the boundaries of viability, the clinical community faces a dual challenge: the technological imperative to intervene and the ethical duty to alleviate suffering. Objectives: This comprehensive review delineates standardized clinical protocols for the management of terminal neonates, emphasizing the strategic shift from aggressive curative interventions to comfort-oriented, holistic care. Methods: An extensive analysis of interdisciplinary management models, nociceptive assessment methodologies, and bioethical frameworks regarding therapeutic futility was conducted. Results: Optimal PPC necessitates early identification of lethal anomalies, the implementation of validated pain assessment tools (NIPS/N-PASS), and the formalization of "humane extubation" protocols. Conclusions: Prioritizing infant dignity and the psychosocial integration of the family unit is paramount. The redirection of care in the neonatal intensive care unit (NICU) should be viewed not as a cessation of treatment, but as a sophisticated optimization of the quality of the end-of-life trajectory.
