Intravascular Lithotripsy-Assisted and Intravascular Ultrasound-Guided Management of In-Stent Restenosis Secondary to Stent Underexpansion
Abstract
A 69-year-old man with diabetes and a history of smoking presented with non-ST-elevation myocardial infarction.Four years previously, he had undergone PCI involving the left anterior descending (LAD) and right coronary arteries (RCA); details regarding stent sizes were unavailable. The patient underwent angiography with revascularization planned due to ongoing chest pain.Angiography revealed chronic total occlusion of the obtuse marginal branch of the left circumflex artery (Fig. 1A).The LAD showed 30% ISR with diffuse midsegment disease (Fig. 1B).The RCA demonstrated 70% mid-to-distal ISR and a significant 90% lesion in the posterolateral (PL) branch (Fig. 1C and D).Revascularization of the RCA ISR and PL lesion under imaging guidance was selected.
