Transverse Myelitis as the Initial Manifestation of Systemic Lupus Erythematosus: A Case Report
Abstract
CASE PRESENTATION►Patient: 35-year-old female with no significant prior medical history.►Onset (1 week prior): Generalized fatigue, migratory polyarthralgia, oral ulcers, hair fall.►Progression (3 days): Progressive, symmetrical lower limb weakness (paraparesis), ascending numbness/tingling.►Acute Worsening (48 hrs prior): Weakness ascended to upper limbs (quadriparesis), urinary retention. OBJECTIVES CONCLUSION INVESTIGATIONS: TREATMENTIn conclusion, this case highlights the importance of a comprehensive diagnostic workup for Systemic Lupus Erythematosus in acute transverse myelitis including CSF analysis, comprehensive serology, and advanced neuroimaging, is essential for accurate diagnosis and timely initiation of appropriate therapy (B-cell depletion therapy) to prevent irreversible neurological disability. RECOMMENDATIONSInvestigate thoroughly: Always look for systemic autoimmune diseases (especially SLE) in TM.Consider Early Escalation: For refractory cases not responding to conventional immunosuppression, consider B-cell depletion therapy (e.g., rituximab) promptly.Multidisciplinary Approach: Essential for optimal management and long-term outcomes.
