Effect of a nurse led model of chronic disease management on cardiorespiratory fitness and health status of patients with atrial fibrillation
Abstract
Objective To explore the effect of a nurse led model of chronic disease management on cardiorespiratory fitness and health status in patients with atrial fibrillation. Methods From January 2023 to August 2023, 58 patients with atrial fibrillation aged between 36 and 75 were admitted to the cardiology ward. They were randomly divided into an observation group and a control group, with 29 cases in each group. All patients received routine treatment and nursing interventions, and those in the observation group received a nurse led model of chronic disease management. Relevant indicators such as cardiorespiratory fitness, quality of life, and drug compliance for patients were measured at hospitalization and 3 months after discharge from hospital, and effectiveness of the interventions were assessed. Results Scores of Atrial Fibrillation-Quality of Life -18 (AF-QoL-18) and 8-item Morisky medication adherence scale (MMAS-8) were increased in both groups at three months after discharge from hospital, and scores of AF-QoL-18 and MMAS-8 in the observation group were higher than those in the control group (P<0. 01). The VO2 peak was significantly higher in the observation group compared to control group at three months after discharge from hospital(P<0. 01). Conclusion The nurse led model of chronic disease management is effective to improve the cardiorespiratory capacity and health status of patients with atrial fibrillation.
