Abstract
Intraoperative hypotension (IOH) is a common complication of general anesthesia and is associated with the development of postoperative organ dysfunction. Despite the existence of clinical guidelines, unified approaches to the prevention and treatment of IOH have not been definitively established and warrant further attention. This review systematizes current evidence on the pathogenesis, risk factors, prognostic significance, and correction methods of IOH in adult patients undergoing major non cardiac surgery. An analysis of 20 literature sources indexed in PubMed and Cochrane Library databases over the period 2000–2025 was performed. The incidence of IOH varies from 20% to 90% depending on threshold values and the monitoring method used. A direct association has been proven between IOH and postoperative complications, including myocardial infarction, acute kidney injury (AKI), cognitive dysfunction, and mortality. Threshold values (absolute SBP < 65–70 mm Hg, a decrease > 30 % from baseline) and episode duration (> 5–10 min) are of critical importance. The review identifies the main prevention strategies: individualized management of patients on renin angiotensin aldosterone system (RAAS) inhibitors, use of dynamic hemodynamic parameters, application of machine learning (Hypotension Prediction Index), and goal directed therapy protocols. IOH is a modifiable risk factor for postoperative complications. The implementation of algorithms for IOH prediction and prevention, as well as the transition from a reactive to a proactive hemodynamic management strategy, should become the standard of anesthetic care.
