Abstract

ABSTRACT: Introduction: Human papillomavirus (HPV) infection is one of the most common sexually transmitted infections, with at least 80% of sexually active individuals becoming infected with at least one genotype of this virus during their lifetime, whereby the expression of oncoproteins and viral persistence represent the ideal conditions for the development of intraepithelial lesions and cervical cancer. The objective of this systematic review was to analyze pathophysiology and updated therapeutic strategies for managing HPV. Materials and Methods: A retrospective systematic review was conducted in accordance with the PRISMA 2020, covering studies published between 2020 and 2025 that were indexed in PubMed, Scopus, and Web of Science. Fifteen articles with sufficient methodological rigor were selected, which were evaluated using the Newcastle-Ottawa Scale (NOS) and the JBI. Results: It was found that viral persistence mediated by the oncoproteins E6 and E7 promotes carcinogenesis, while prophylactic vaccination reduces the incidence of infection and the occurrence of high grade squamous intraepithelial lesions by 95%. Discussion: Photodynamic therapy and immunomodulators demonstrated better conservative outcomes compared to traditional surgical techniques. Conclusions: A comprehensive approach to HPV should prioritize primary prevention, early diagnosis, and individualized therapeutic strategies based on scientific evidence.
