Abstract

INTRODUCTION . Resection of insular lobe tumors increases the survival rate and quality of life of patients, but it is believed that with an increase in resection volume, the risk of developing additional neurological deficits increases. AIM . To identify the factors influencing the choice of surgical tactics and the degree of resection of tumors of the insular lobe. MATERIALS AND METHODS . A retrospective study of 119 cases of surgical treatment of the insular lobe tumors was conducted from 2015 to 2023 at the Federal Neurosurgical Center in Novosibirsk. The volume of tumor resection and its localization were evaluated. The relationship with postoperative complications has been established and the factors that aggravate the condition of patients after surgical treatment have been identified. RESULTS. Women are more susceptible to the disease, with a debut in the form of epileptic seizures. In 77.1 % of cases, the tumor spreads beyond the insular lobe. The volume of resection of malignant neoplasms turned out to be greater than that of benign ones, due to better visualization of the tumor boundaries. Motor disorders were noted in 15.9 % of cases, speech disorders in 18.4 %. CONCLUSION . The factors that must be taken into account when performing surgical treatment of insular tumors are the localization and degree of malignancy of the tumor. Negative factors are: the growth of the tumor beyond the insular lobe and the desire for total removal of the tumor. Accordingly, the removal of a tumor in the insular region should be a balanced decision using modern methods of preoperative preparation, intraoperative monitoring and with the removal of the maximum possible volume of the tumor while preserving functionally important areas of the brain.
