Abstract

Hysterectomy is one of the most common gynaecological surgeries performed worldwide; however, the optimal surgical route for benign indications remains controversial, with a clear predominance of TAH over VH in India, despite existing evidence for superior postoperative recovery after VH.This study tries to address this knowledge gap by comparing comprehensive postoperative recovery profiles between VH and TAH in a controlled clinical setting.A prospective comparative study was conducted involving 60 patients scheduled for elective hysterectomy, who were systematically allocated into two groups: Group I (VH, n=30) and Group II (TAH, n=30).The primary outcome measures included operative time, estimated blood loss, post-operative pain, complication rates (SSI), and patient satisfaction levels. The demographics showed no intergroup differences continuously (p>0.05).Operative times were similar: VH 78.6±14.2min vs TAH 82.9±16.1 min (p=0.18).VH demonstrated reduced blood loss (225.4±58.6 mL vs 255.9±64.2mL, p=0.12), lower pain (0.14±0.28 vs 1.18±0.45,p=0.12), zero SSI (vs 16.7% TAH, p=0.012), and higher satisfaction: 60% very satisfied vs 43.3% (p=0.30).Wound healing was similar (mild changes were predominant with p>0.05), with VH showing favorable trends.VH offers safety profile equivalent to TAH but with superior recovery profile (reduced blood loss, pain, infection; faster mobilization/discharge) for suitable benign conditions compared to TAH.These data suggest the need for VH training for optimum utilization in resource-constrained situations.
