Abstract

Perioperative care spans preoperative preparation, intraoperative management, and postoperative recovery, with the overarching goal of optimizing safety, outcomes, efficiency, and quality. Beyond cost reduction and institutional performance, high-functioning perioperative systems elevate the patient experience. Interventions such as streamlined scheduling and enhanced recovery after surgery (ERAS) protocols, supported by interdisciplinary collaboration, have reduced surgical delays, hospital length of stay, and readmissions. Advances in anesthetic techniques and monitoring further promote safety by lowering surgical site infection risk, minimizing anesthesia-related morbidity, and enhancing comfort during recovery. A patient-centered model, including preoperative clinics for risk stratification, comorbidity management, and patient engagement, aligns surgical success with functional recovery and improved quality of life. Operational strategies using lean and six sigma methodologies, coupled with performance metrics such as on-time starts and turnover time, strengthen efficiency, satisfaction, and financial sustainability. Yet, without a supportive work culture, such initiatives often falter. Building trust, teamwork, and engagement fosters a positive environment where improvement efforts in clinical pathways, operations, and culture can succeed. Ultimately, aligning efficiency with safety allows perioperative care to achieve its core mission: delivering high-quality, patient-centered surgical outcomes while ensuring healthcare system sustainability.
