Abstract

Abstract: Pulmonary embolism (PE) is a life-threatening emergency whose diagnosis relies on clinical probability, biomarkers, and imaging. D-dimer offers high negative predictive value, yet isolated interpretation may mislead clinical judgment. We report a 59-year-old male with sudden precordial pain, tachycardia, and hypoxemia, with normal D-dimer on admission. Point-of-care echocardiography revealed right ventricular overload, prompting a complete workup and PE confirmation by CT pulmonary angiography. This case highlights the limitations of current diagnostic algorithms and reinforces the critical role of bedside ultrasound for timely decision-making in emergency settings.
