Familial POAG to Refractory Glaucoma: A Case Report on a Low-Cost Glaucoma Implant
Abstract
This case report contributes new evidence on the clinical effectiveness of the low-cost Virna Glaucoma Implant (VGI) for refractory glaucoma in a patient with strong familial POAG history an important scenario frequently encountered in low- and middle-income countries. It highlights that after two failed trabeculectomies and uncontrolled IOP of 43.4 mmHg, VGI implantation produced rapid IOP stabilization (10 mmHg on Day 18 and 19 mmHg on Day 32) with preserved visual acuity and no complications. The report strengthens existing literature demonstrating that non-valved, affordable glaucoma drainage devices can achieve IOP reduction comparable to Ahmed or Baerveldt implants, while remaining financially accessible in resource-limited settings. This manuscript expands the global understanding of VGI as a viable, cost-effective GDD option for refractory glaucoma and underscores the importance of accessible surgical glaucoma care for patients with limited economic resources.
