Challenges in Translating and Adapting ADOS-2: Legal, Ethical, and Methodological Considerations
Abstract
Background: The Autism Diagnostic Observation Schedule, Second Edition (ADOS-2), is widely regarded as the gold standard observational tool for the diagnosis of autism spectrum disorder (ASD) and is used internationally. The growing global demand for ASD assessment has led to multiple translation and adaptation efforts; however, this process involves complex legal, ethical, and methodological challenges that remain insufficiently documented in the peer-reviewed literature. Methods: This narrative and critical review examined peer-reviewed studies and international guidelines addressing the translation and adaptation of the ADOS-2. Literature was retrieved from PubMed, Scopus, PsycINFO, and Google Scholar, alongside guidance from the International Test Commission (ITC), the World Health Organization (WHO), and Western Psychological Services (WPS). The review focused on identifying recurring challenges, rather than proposing formal guidelines. Results: Only six peer-reviewed studies met the inclusion criteria, despite the widespread international use of the ADOS-2. This limited number reflects the limited transparency of adaptation processes in the academic literature. Recurring challenges included copyright and licensing requirements, ethical considerations such as informed consent and cultural fairness, maintenance of psychometric integrity, and difficulties in achieving conceptual equivalence across cultures. The findings also show that adaptation guidance often relies on proprietary or grey literature. Conclusions: The adaptation of the ADOS-2 extends beyond linguistic translation and requires careful attention to legal, ethical, and methodological dimensions. Using ADOS-2 as a case example, this review highlights a gap between its widespread clinical use and the limited peer-reviewed evidence on cross-cultural adaptation.
