Abstract

This cross-sectional study, conducted at Darul Sehat Hospital , karachi Pakistan , explored the relationship between the LH/FSH ratio and endocrine parameters in women with PCOS diagnosed by Rotterdam criteria. Among 130 reproductive-aged women, 20.8% had an LH/FSH ratio >2. Those with elevated ratios demonstrated significantly higher mean TSH levels (p=0.049), highlighting a potential link between gonadotropic imbalance and thyroid dysfunction. Although prolactin and insulin showed no significant mean differences between groups, a weak but significant positive correlation was observed between prolactin and LH/FSH ratio (r=0.191, p=0.029). These findings underscore the importance of routine thyroid assessment in PCOS patients, particularly those with elevated LH/FSH ratios, to optimize comprehensive endocrine management . This study highlights the importance of routine thyroid screening in PCOS patients with elevated LH/FSH ratios, enabling early detection of thyroid dysfunction and improving individualized hormonal and reproductive management strategies.
