Abstract
Background: Schizophrenia is a chronic psychiatric disorder with a multifactorial etiology, including genetic contributions. Genetic polymorphisms involved in dopamine signaling and nucleotide metabolism, such as nt5c2 and DRD2, have been suggested to be associated with clinical manifestations and cognitive function in patients with schizophrenia. Objective: This study aimed to describe the demographic characteristics of Batak ethnic patients with schizophrenia and healthy controls, and to analyze the association of rs11191580 nt5c2 and DRD2-141C polymorphisms with schizophrenia phenotype, symptom severity assessed by PANSS, and cognitive function assessed by MoCA-INA. Methods: A cross-sectional study was conducted involving Batak ethnic patients with schizophrenia and healthy controls. Genotyping of rs11191580 nt5c2 and DRD2-141C was performed and analyzed in relation to schizophrenia phenotype, PANSS scores, and MoCA-INA scores. Statistical analyses included Chi-square, Mann–Whitney U, and Kruskal–Wallis tests, with p < 0.05 considered statistically significant. Results: A significant difference in rs11191580 nt5c2 genotype distribution was observed between patients and controls (p = 0.022); however, no significant association was found with PANSS scores or MoCA-INA scores and interpretations (p > 0.05). The DRD2-141C polymorphism showed a significant difference in genotype distribution between patients and controls (p < 0.001), but was not associated with PANSS-based symptom severity (p = 0.879). Cognitive analysis revealed no significant difference in MoCA-INA scores among DRD2-141C genotypes (p = 0.213), although a significant difference was observed in the distribution of MoCA-INA cognitive categories (p < 0.001). Conclusion: The rs11191580 nt5c2 and DRD2-141C polymorphisms demonstrated different genotype distributions between Batak ethnic patients with schizophrenia and healthy controls. However, these polymorphisms were not consistently associated with symptom severity or numerical cognitive scores.
