Abstract

Heart failure (HF) management has advanced significantly over recent years, driven by innovations in pharmacotherapy, device-based interventions, and remote monitoring technologies. These developments have collectively improved survival, reduced hospitalizations, and enhanced quality of life for patients across the HF spectrum. New pharmacological agents—including neprilysin inhibition combined with RAAS blockade (sacubitril/valsartan), sodium–glucose cotransporter-2 inhibitors, direct myosin activators such as omecamtiv mecarbil, and soluble guanylate cyclase stimulators such as vericiguat—target diverse pathways involved in cardiac remodeling, hemodynamic regulation, and myocardial energetics. While agents such as serelaxin and ularitide remain under evaluation, these newer therapies mark a shift toward mechanism-driven treatment. Parallel technological advances have transformed HF care through both intracardiac and extracardiac innovations. Cardiac contractility modulation with the Optimizer Smart System, biventricular pacing for resynchronization, and implantable cardioverter-defibrillators have become critical tools in reducing arrhythmia-related mortality and improving functional capacity. Mechanical circulatory support devices, including left ventricular assist devices and Impella systems, offer life-saving therapy for advanced HF and serve as essential bridges to transplantation. Emerging extracardiac devices—such as vagal nerve stimulators (Vitaria), baroreflex activation systems (Barostim Neo), splanchnic nerve ablation (Satera system), and enhanced external counterpulsation—expand therapeutic options by modulating autonomic tone, hemodynamics, and intravascular volume. Together, these pharmacological and technological innovations support a more personalized and proactive approach to HF management, enabling earlier detection of decompensation, improved symptom control, and better long-term outcomes. Continued research will further refine these therapies and expand their applicability, solidifying their role in comprehensive, patient-centered HF care.
