Abstract

ABSTRACT Introduction: Low health literacy represents a major challenge for clinical communication in Primary Health Care, especially among older adults and people with chronic noncommunicable diseases, affecting understanding of guidance, treatment adherence, and self-care. Objective: To report the experience of a multiprofessional Primary Health Care team in implementing clinical communication strategies aimed at users with low health literacy. Method: This is an experience report developed in a Family Health Unit in the municipality of Anápolis, Goiás, Brazil. Articles published between 2020 and 2025 were searched in the PubMed, SciELO, and Google Scholar databases. Results and discussion: The strategies adopted by the team promoted greater understanding of health information, strengthening of the professional–user bond, and an increased perception of support, especially among individuals with lower educational levels and limited digital skills. Conclusion: The adoption of communication strategies based on health literacy proved to be feasible and relevant in Primary Health Care, contributing to improved quality of care and reduction of communication barriers. Strengthening these practices requires continuous team training and organizational support. Keywords: Health literacy; health communication; primary health care; chronic disease.
