Abstract

Patient safety has become a central component of healthcare quality over the last decades, recognized as a fundamental pillar for reducing risks and adverse outcomes. This integrative review aimed to analyze advances, barriers, and future perspectives in patient safety, considering both national and international scientific literature. The study involved a systematic search of articles in major health databases, selecting publications that addressed strategies, interventions, and organizational practices aimed at minimizing preventable errors in clinical care. The findings demonstrate that advances in patient safety are closely linked to the implementation of standardized protocols, interdisciplinary teamwork, technological innovations, and continuous professional education. Nevertheless, persistent barriers remain, including communication failures, resistance to a safety culture, workforce turnover, and structural limitations in healthcare institutions. Evidence also highlights that the active involvement of patients and family members, alongside collaborative professional practices, significantly contributes to reducing errors and improving healthcare outcomes. The review emphasizes the importance of a systemic and integrated approach that combines organizational policies, technological resources, and cultural transformation to consolidate patient safety effectively. Furthermore, it identifies research gaps, suggesting the need for continuous monitoring of safety interventions and the promotion of strategies that strengthen institutional safety culture. The results underscore that patient safety is not only a clinical imperative but also a societal responsibility, requiring commitment from all healthcare stakeholders. This integrative review provides a comprehensive understanding of the current state of patient safety and offers recommendations for practice, policy, and future research aimed at sustaining safe and high-quality healthcare services.
