REUNIÃO DE EQUIPE E COORDENAÇÃO DO CUIDADO NA ATENÇÃO PRIMÁRIA À SAÚDE: RELATO DE EXPERIÊNCIA DE UMA RESIDENTE EM MEDICINA DE FAMÍLIA E COMUNIDADE
Abstract
Primary Health Care (PHC) is the preferred gateway and communication center of the Unified Health System (SUS), with the Family Health Strategy (FHS) as its priority organizational model. To operationalize the guidelines for care coordination and integrality, team meetings emerge as an indispensable tool for local planning and Permanent Education in Health (PEH). The objective of this study is to report the experience of a Family and Community Medicine resident regarding the implementation of team meetings as a care coordination strategy in a Family Health Unit in Anápolis, Goiás. This is a qualitative study, specifically an experience report, based on monthly meetings that began in March 2024. Data collection instruments included participant observation and documentary research in minute books and in the municipal electronic medical record system Celk Saúde©. The results demonstrate that the meetings favored multiprofessional integration and the collective construction of care plans, especially in complex cases of mental health and chronic conditions. The active participation of Community Health Agents (CHA) enhanced territory diagnosis and active screening. It is concluded that the team meeting is a fundamental tool for work process organization, strengthening bottom-up planning and consolidating the health unit as a strategic setting for in-service training.
