Succeeding with AI in healthcare: A practical startup framework
Abstract
Artificial intelligence (AI) is increasingly seen as essential for addressing rising demand, workforce shortages, and efficiency challenges in healthcare. Yet, AI healthcare startups operate within a uniquely complex environment defined by strict regulation, ethical risks, data scarcity, and the need to earn trust in a highly sensitive sector. Motivated by these challenges, this thesis examines how early-stage startups can navigate the dual complexity of both healthcare systems and AI development. The study uses a qualitative research design based on a structured review of academic literature, industry reports and an interview-based study of 55 AI healthcare startup founders. Insights from healthcare management, AI ethics, clinical research, and entrepreneurship are synthesized to identify recurring challenges, stakeholder expectations, and the factors that shape the adoption and sustainability of AI innovations in clinical settings. The analysis shows that startups face significant barriers including privacy constraints, limited access to high-quality data, algorithmic bias, organizational readiness gaps, and persistent trust deficits among clinicians, while also operating in an environment increasingly open to AI-driven solutions. The thesis concludes by proposing a practical framework that emphasizes identifying a real clinical need, building multidisciplinary teams, developing trustworthy and bias-aware AI models, and engaging clinicians early. This framework addresses a recognized research gap and offers actionable guidance for founders, investors, and stakeholders supporting innovation in healthcare.
