Abstract

Objectives: Stroke is the second leading cause of death and the leading cause of disability worldwide. Atherosclerotic stenosis of the internal carotid artery plays a significant role in its etiology, accounting for approximately 20% of ischemic strokes. The main objective of the dissertation was to retrospectively evaluate the results of carotid artery stenting (CAS) in patients with laboratory-confirmed antiplatelet therapy efficacy and to identify risk factors for periprocedural complications. The main hypothesis tested whether testing and individualization of antiplatelet therapy reduce the incidence of thromboembolic complications and new ischemic lesions (NIL). The secondary objectives were to compare the efficacy of clopidogrel and ticagrelor and to assess periprocedural complications of CAS in the context of international studies. Methods: Retrospective observational data sets of patients treated at the Radiology Clinic of Masaryk Hospital in Ústí nad Labem between 2014 and 2023 were analyzed. The effectiveness of antiplatelet therapy was tested using light aggregation and the Multiplate® method. The incidence of clopidogrel resistance and the effectiveness of ticagrelor as an alternative were evaluated. The analysis of periprocedural complications included 241 patients with 248 treated...
