Abstract

Superficial dorsal penile vein thrombophlebitis is a benign, typically self-limited condition involving the superficial dorsal vein of the penis. Although uncommon, it is likely under-recognized and may cause significant patient anxiety. We report the case of a 37-year-old man with a persistent, sclerotic dorsal penile cord and erection-related pain that began after sexual intercourse three years prior to presentation. Given the chronicity of symptoms, excision of the superficial dorsal vein was performed. Histopathology demonstrated focal intimal thickening without luminal obstruction and no identifiable residual thrombus, consistent with chronic superficial venous changes. Diagnosis is primarily clinical, with color Doppler ultrasound useful when diagnostic uncertainty exists. First-line management includes reassurance, temporary sexual abstinence, and non-steroidal anti-inflammatory drugs. Surgical management (thrombectomy or vein excision) may be considered in persistent or refractory cases.
