Abstract

Objective: to present the information of a clinical case where the presence of the QD prevented the normal eruption of the first permanent lower first molar and how it was approached. Methodology: this case was developed according to CARE (case report guidelines). We present a minor patient who was diagnosed with a dentigerous cyst associated with the crown of the lower first molar, detected as a radiographic finding, and subsequently underwent surgery with complete enucleation of the lesion without extraction of the involved tooth, in addition to postoperative follow-up. Results: In the clinical case, complete enucleation of the dentigerous cyst in dental organ 3.6 was performed without the need for its extraction. During surgery a well-demarcated cystic capsule of approximately 12 mm in diameter was identified and removed in its entirety. The intervention allowed the affected tooth to be preserved. The postoperative follow-up at 21 days showed the presence of the dental organ, confirming the beginning of the physiological eruption process. Conclusion: The importance of timely diagnosis in the absence of eruption of a permanent tooth is emphasized in order to prevent malocclusions. The treatment chosen without extraction of the affected tooth proved to be effective and conservative, favoring the passive eruption of the compromised dental organ. In addition, this therapeutic alternative is useful in pediatric patients when the root has not completed its development, since it preserves the bone architecture and avoids unnecessary orthodontic interventions.
